By now you have noticed that the Washington State Employment Security Department has revised and renamed several common unemployment forms. To help you easily identify each form and determine its purpose, we have created a quick video overview, below. We have also provided samples of each form below, with a brief description of the form’s purpose.
- Request for Separation Information. Used to gather employment history.
- Wage Verification. Used to verify the wages reported by the claimant.
- Benefit Charging Notice. Shows the portion of benefits the employer is responsible for and includes an employment history request.
- School Employer Questionnaire. Used to determine whether there is a contract or reasonable assurance.
Remember the two keys to a successful response to an unemployment claim are completeness and timeliness.
