Examples of Notification Forms

The following forms notify you that a former employee of yours has filed an unemployment claim. Wages reported by your district for this individual have been reported in the base year.

Benefit award decisions are based on the information you provide on these documents. They must be returned in a timely manner.

According to a new law, if a district or district representative have a pattern of significantly contributing to benefit overpayments by failing to respond timely or adequately to information requests without good cause, you won’t be eligible for benefit-charge relief on the relevant claims. Translation: if you are late, you may not get a credit or repayment.
