Benefit award decisions are based on the information you provide on these documents. They must be returned in a timely manner.
According to a new law, if a district or district representative have a pattern of significantly contributing to benefit overpayments by failing to respond timely or adequately to information requests without good cause, you won’t be eligible for benefit-charge relief on the relevant claims. Translation: if you are late, you may not get a credit or repayment.
Notice to Employer – EMS 5361
- Reason claimant gave for no longer working
- Sent to final employer
- Sent to other employers if there is a question about why the person is no longer working

- Employer may be charged for benefits paid
